On completing and submitting this form you are agreeing to these terms:

• I am confirming I would like to reserve a Minton Manor Labradoodle Puppy, I have completed this reservation form truthfully and read and agreed to the code of ethics.

• I would like to make a reservation for puppy for of £300 and I understand this amount will be deducted from the total sale cost.

• I understand that this is a reservation fee and that it is non-refundable.

• I understand that the total balance of £2700 (for Pet Puppies) will be due to be paid on viewing day when the puppies are 5 weeks old.

• I understand that a payment of £7000 will due if the pet puppy is not spay/neutered as per the contract, evidence of the spay/neuter needs to be sent to Minton Manor Labradoodles or otherwise a final amount will be charged.
